Today we are sharing a blog about a patient on the other end of that spectrum.
We also get the question, “Am I too young?” for blepharoplasty. And the answer, as this patient’s story makes clear, is the same.
Age has nothing to do with it.
She Was in Her 30s With Under Eye Bags that Bothered Her.
This patient is a young, healthy woman in her 30s who came to our Fullerton office for a consult regarding her lower eyelids.
She had tried every cream and product available for her under eye bags without any effect. She tried getting more sleep, staying well hydrated, taking allergy medication but nothing seemed to work.
Looking at her before photo, the lower eyelid fat herniation is clearly visible beneath both eyes. The fat that naturally cushions the eye had pushed forward through the thin membrane beneath the lid, creating a persistent puffiness and shadow that made her look tired regardless of how rested she actually was. Her upper eyelids are completely unaffected. This was purely and specifically a lower eyelid concern, and it was one that no amount of sleep, skincare, or time was ever going to resolve on its own.
Why Under Eye Bags Can Appear in Your 20s and 30s
One of the most common misconceptions about under eye bags is that they are a consequence of aging. That they appear when you get older. That if you have them young it must mean you are not taking care of yourself.
None of that is accurate.
The development of lower eyelid fat herniation is largely determined by genetics. The strength and thickness of the orbital septum, which is the thin membrane that holds the fat cushion around the eye in its proper position, is inherited. If your parents or grandparents had prominent under eye bags at a young age there is a meaningful chance you will too, regardless of how well you sleep or how carefully you take care of your skin.
For many patients the fat herniation begins developing in their 20s and becomes clearly visible by their 30s. By the time they come in for a consultation they have often spent years assuming it would improve with better habits, spending money on products that were never designed to address the actual problem, and feeling frustrated every time they looked in the mirror.
The truth is that once fat has herniated forward through a weakened orbital septum, it does not go back. Lower blepharoplasty is the only treatment that genuinely and permanently addresses the underlying structural cause.
The Procedure: Scarless Lower Blepharoplasty
For this patient Dr. Peyman performed a lower blepharoplasty using a transconjunctival approach. This means the incision was made on the inside of the lower eyelid rather than on the visible outer skin. The herniated fat was sculpted and repositioned through that hidden internal incision, leaving absolutely no external scars whatsoever.
This approach is particularly well suited to younger patients for several reasons. Because there is no external incision there is no visible scar on the outer skin of the lower lid. The recovery tends to be smooth and manageable. And because younger patients generally have better skin elasticity the lower lid contour redrapes beautifully following transconjunctival blepharoplasty without the need for any additional skin tightening.
The procedure was performed under local anesthesia right here at our Fullerton office. She was comfortable throughout, went home the same day, and quickly recovered within a week.
The After: Rejuvenated Eyes
Looking at her photo following surgery, the transformation has naturally addressed her concern.
The fat herniation that had been creating puffiness and shadowing beneath both eyes is gone. The transition from her lower lid to her cheek is now smooth and seamless. Her eyes look brighter and more alert. And most importantly, she likes what she sees now in the mirror, a confident young woman whose eyes now reflect the energy and vitality that were always there.

From 30s to 80s and Inbetween: What These Two Stories Tell Us
When we step back and look at both of these patient stories side by side a clear and important truth emerges.
The man in his 80s who assumed he might be too old and the woman in her 30s who might have assumed she was too young both came to our Fullerton office with genuine concerns about their eyelids. Both tolerated the in office procedure well, recovered well, and left with natural, rejuvenated eyelids. This shows that age is not a significant factor in and of itself.
The real question is always the same regardless of whether a patient is 30 or 80. Is this a real concern affecting how you look and feel? Is there a structural cause that will not resolve on its own? FAQ: Lower Blepharoplasty for Younger Patients
Is there a minimum age for lower blepharoplasty?
There is no universal minimum age for lower blepharoplasty. Candidacy is determined by the presence of a genuine structural concern, overall health, and a thorough evaluation rather than age alone. Patients in their 30s can and do have successful lower blepharoplasty when fat herniation is clearly present and affecting their self esteem or confidence.
Why do I have under eye bags if I am young and healthy?
Under eye bags caused by fat herniation are largely genetic. The strength of the membrane holding the orbital fat in place is inherited and has very little to do with sleep, hydration, diet, or skincare. If your parents had prominent under eye bags at a young age you may be predisposed to the same condition.
Will eye creams fix my under eye bags?
Eye creams can hydrate and brighten the skin around the eyes but they cannot address herniated orbital fat. Once the fat has moved forward through a weakened membrane, topical products have no mechanism to reverse it. Fillers may be able to mask small amounts of fat but lower blepharoplasty is the longest lasting and, commonly, the most effective treatment that addresses the structural cause.
What is a transconjunctival blepharoplasty?
A transconjunctival blepharoplasty is a lower eyelid procedure where the incision is made on the inside of the lower lid rather than the outer skin. This leaves no visible external scarring and is particularly well suited to younger patients with good skin elasticity. It also greatly decreases the down time associated with the procedure.
How long is the recovery for lower blepharoplasty?
Most patients return to light daily activities within one to two weeks. Visible swelling and bruising typically resolve within two to three weeks with final results visible by a month.
Are the results permanent?
Lower blepharoplasty results are long lasting. Once the herniated fat is sculpted and/or repositioned, it does not return. Most patients enjoy their results for the rest of their lives.
